Documentation and fee requirements for applicants
The OTBNZ will begin processing your application once it has received all the required documents and fees below. Photocopied documents must be certified by a Justice of the Peace, solicitor, or notary public.
- Registration application fee
- Competence assessment application – not required from recent New Zealand graduates
- Cultural competence application – not required from recent New Zealand graduates
- Practising certificate application
- Practising certificate fee – not required from recent New Zealand graduates
- Curriculum vitae – not required from recent New Zealand graduates
- 2 passport-sized photographs (with your name and the date of birth on the back of each one).
Certified copies of:
- Birth certificate
- Passport or NZ Driver Licence
- Any legal evidence of name change (e.g. marriage certificate)
- Occupational therapy qualification certificate – not required from recent New Zealand graduates or applicants under the Trans Tasman Mutual Recognition Act (TTMRA) 1997
- Course description – not required from recent New Zealand graduates or applicants under the TTMRA
- Academic syllabus of your occupation therapy qualification – not required from recent New Zealand graduates or applicants under the TTMRA
- Academic transcript – not required from recent New Zealand graduates or applicants under the TTMRA
- Certificate or letter of good standing from every registering authority with which you have been registered (if applicable) – no older than six months on receipt by the OTBNZ
- New Zealand police report (if applicable) – no older than six months on receipt by the OTBNZ
- Overseas police reports (if applicable) – no older than six months on receipt by the OTBNZ
- IELTS results (if applicable) – no older than 24 months on receipt by the OTBNZ or 3 references attesting specifically to your English language competence
- Three references – not required from recent New Zealand graduates.
Signed declaration from you stating:
- that you do not suffer from any mental or physical condition that may affect your ability to communicate or to perform the functions of occupational therapy
- that you are aware that the information collected with the application will be shared with the Ministry of Health
- that you have no pending criminal charges
- that you are not currently under investigation
- that all information provided is true and correct.
