Changing details on the register

Registered practitioners may need to update their OTBNZ records to reflect an address change, a name change, or to add a qualification to their record on the Register.  This page provides information on how to make these changes.

Change of address
Pursuant to the Health Practitioners Competence Assurance Act 2003 (HPCAA), it is the responsibility of all registered practitioners to notify the OTBNZ promptly of any change to their residential, postal or work addresses.  Practitioners may update their details on the practitioner log on section of this website.  Registered practitioners who need to amend their address details but do not have log on access should contact the OTBNZ.
 
Change of name
Practitioners who wish to make a change to their name on the Register are required to provide a certified copy of the legal evidence of the name change (e.g. a marriage certificate). 
 
If the requested name change is to revert to a previous name (e.g. a maiden name), please contact the OTBNZ as it is possible that the required documentation is already on file.
 
Adding a qualification
The HPCAA requires the OTBNZ to list the qualifications of occupational therapists on the Register.  The Register displays the qualification that was approved by the OTBNZ for registration in New Zealand.  Practitioners may add:
  • any additional New Zealand qualification that is level 7 (Bachelor Degree) or above
  • any additional overseas qualifications such as a Baccalaureate Degree, Postgraduate Diploma, Master Degree or PhD.

Only formal registration, graduate and postgraduate qualifications will be displayed on the Register.

For the OTBNZ to add a new qualification to the practitioner’s record, the practitioner is required to supply a certified copy of the qualification certificate.
