Self-declaration - practising certificate/recertification application form

The self-declaration forms part of the practising certificate application and is submitted when applying for a practising certificate.  This self-declaration provides the OTBNZ with information about recent practice, any recent overseas residency, any recent convictions, any current disciplinary proceedings or investigations relating to competence or conduct, or any issues that might impact on fitness to practice. Applicants are also asked to confirm that they are participating in the Continuing Competence Framework for Recertification. The OTBNZ may contact the practitioner if more information is required in relation to any aspect of the declaration.
